Cholangiocarcinoma miscoding in hepatobiliary centres

چکیده
Introduction Cholangiocarcinoma (CCA) are sub-divided into intrahepatic (iCCA) or extrahepatic (eCCA). eCCA further subdivided perihilar (pCCA) and distal (dCCA). Current previous versions of the WHO International Coding Disease Oncology classifications (ICD) have separate topography codes for iCCA eCCA, but none pCCA. Over recent decades, multiple studies report rising incidence rates with declining without reference to We hypothesised lack a specific code pCCA has led errors CCA coding, specifically miscoding as iCCA. Methods Clinical notes cases coded hepatobiliary carcinoma using ICD-10 criteria (C22.1/Intrahepatic Bile Duct carcinoma, C24.0/Extrahepatic C23X/Malignant Neoplasm Gall Bladder, C22.0/Malignant Liver Cell Carcinoma) over 2 year period (2015–2017), were reviewed by two independent clinicians at three UK regional HepatoPancreatoBiliary centres. The agreed final diagnosis was compared originally allocated code. Results Of 625 fully reviewed, 226 C22.1/iCCA. 98 (43%) these true correctly, while 76 (34%) actually 92% all incorrectly Conclusion coding misclassification in HPB centres is common, particularly pCCA, which commonest form CCA, This may be contributing apparent Our findings confirm need implement distinct topographical iCCA, dCCA future iterations ICD.

Secondary sclerosing cholangitis in localized hepatobiliary tuberculosis simulating cholangiocarcinoma: a rare case report
BACKGROUND Hepatobiliary tuberculosis includes miliary, tuberculous hepatitis or localized forms. The localised form is extremely uncommon and can mimic malignancy. Still rarer is its presentation as sclerosing cholangitis. CASE PRESENTATION A 50 year male presented with acute onset jaundice, significant weight loss and elevated liver enzymes with clinico-radiological suspicion of cholangioca...
متن کاملHepatobiliary scintigraphy in 2014.
Cholescintigraphy with (99m)Tc-hepatobiliary radiopharmaceuticals has been an important, clinically useful diagnostic imaging study for almost 4 decades. It continues to be in much clinical demand; however, the indications, methodology, and interpretative criteria have evolved over the years. This review will emphasize state-of-the-art methodology and diagnostic criteria for various clinical in...

AIMS To determine the effectiveness of self-audit tools designed to detect miscoding, misclassification and misdiagnosis of diabetes in primary care. METHODS We developed six searches to identify people with diabetes with potential classification errors. The search results were automatically ranked from most to least likely to have an underlying problem. Eight practices with a combined popula...
متن کاملSeromucoid in hepatobiliary disease.
A N INCREASE IN CIRCULATING carbohydrate-rich proteins has been known for some time to occur in diseases associated with neoplastic or inflammatory processes. These substances have been variously termed polypeptides (1-4), proteoses (5), mucoid-like material (6), seromucoid (7-10), and mucoprotein (11-15). It was only recently that the occurrence of a pathologic depression of the seromucoid (SM...

Only a few years after the introduction of laparoscopic cholecystectomy in the late 1980s, surgeons declared the laparoscopic procedure as the gold standard for symptomatic gallstone disease. Several studies have shown the efficacy and safety of the procedure as well as the advantages such as reduced hospital stay, earlier recovery, less intraabdominal adhesions and a better cosmetic outcome [1...
